General

When making an appointment, you agree to the general terms and conditions.
The therapist is obliged to act in accordance with the professional code.
The client has the right to inspect his/her own file.
The client undertakes to cancel an appointment in good time (at least 24 hours in advance).
The therapist undertakes to provide information regarding the treatment to the client in all phases of the treatment.

The therapist will maintain confidentiality with regard to data provided by the client for the file (with the exception of data about which the client has declared that he/she does not object to the use for statistical or treatment-related purposes).

The therapist may not perform certain treatments without the client’s permission.

The therapist undertakes to correctly refer the client to a fellow therapist or a doctor if his/her treatment is not suitable and/or sufficient.

The client is prepared to follow the advice given by the therapist and, if he/she deems this necessary, to visit a fellow therapist or a doctor. The therapist is required to have taken out business liability insurance.

Termination of treatment can be done at any time by mutual consent.

 

House rules

Acupuncture and additional treatments by the therapist are a supplement to regular medicine and not a replacement. The client should always consult the general practitioner in case of health complaints.

The treatment takes place in a separate room in which only the therapist and the client are present. If a third party is present (e.g. an intern or colleague), the therapist will ask the client for permission before the treatment.

The treatment plan and the method are discussed with the client before the treatment. If there are objections to specific actions, the client must indicate this immediately. If the client agrees to the treatment plan, a treatment agreement is concluded.

The therapist acts with great care, but the chance of bruising, swelling or damage to the skin cannot be completely ruled out. If this should happen, the therapist is responsible for correct care. Furthermore, this falls under the client’s own risk.

The therapist and client must behave according to general standards of decency. In the event of offensive behavior, the therapist has the right to deny the client access to the building. In case of calamities, the client must follow the instructions of the therapist or the competent authority (fire brigade, police). The therapist is not responsible for theft or damage of property in or around the building.

 

Privacy policy

For proper treatment, it is necessary for the treating therapist to create a file. This is also a legal obligation imposed by the Medical Treatment Agreement Act (WGBO). Your file contains notes about your state of health and information about the examinations and treatments performed.

The file also contains information that is necessary for your treatment and that the treating therapist, after your explicit consent, has requested from another healthcare provider, such as your general practitioner.

We do our best to guarantee your privacy. This means, among other things, that we:

  • handle your personal and medical data with care;
  • ensure that unauthorized persons do not have access to your data.

Your treating therapist is the only one who has access to the data in your file.

Your treating therapist has a legal duty of confidentiality (professional secrecy).

The data from your file can also be used for the following purposes:

 

To inform other healthcare providers, for example when the therapy has been completed or when referring to another practitioner. This only happens with your explicit consent.

For use during observation, during my absence.

For anonymised use during peer review.

A small part of the data from your file is used for the financial administration, so that the treating therapist or his administrator can draw up an invoice.

If the treating therapist wants to use your data for another reason, he will first inform you and explicitly ask for your permission.

 

Privacy on the healthcare invoice

The healthcare invoice that you receive contains the data requested by the health insurer, so that you can declare this invoice with your health insurer.

 

  • Your name
  • Your customer number and/or your date of birth
  • the date of the treatment
  • Consultation: Acupuncture performance code 24104
 

Payment conditions

Appointments can be cancelled free of charge up to 24 hours in advance. For appointments that are cancelled within 24 hours of the appointment, the full rate of 75 euros applies. This also applies to first appointments.

Preferably pay by debit card. If (exceptionally) payment is made by bank transfer, please pay the invoice no later than 31 days after the invoice date.

If you have not paid, or not paid in full, within the stated period, you will be in default by operation of law from the 31st day. The treatment will then be stopped until the invoice has been paid.

If the practitioner sends you a payment reminder, an administration fee of € 10.00 may be charged for this.

If you are in default, the practitioner has the right to take collection measures or to charge third parties with this. All costs associated with the collection (including extrajudicial collection costs) will be charged to you.

Health insurance

Freeman Works is registered with the KAB, Koepel Alternatieve Behandelwijzen, and with the Dutch Association for Traditional Chinese Medicine Zhong.

 

Acupuncture treatments generally fall under complementary care within a supplementary insurance. It is the client’s own responsibility to check what amount is reimbursed per day and per year. The client must also ensure that the invoices are declared to the health insurance company.

 

Disciplinary regulations and complaints

The therapist is subject to the disciplinary law provided by the KAB (Koepel Alternatieve Behandelwijzen). link to KAB

 

It is preferable to discuss complaints with the therapist in the first instance.
